Stephanie : Planning for unaccompanied leave

Mental Health Nursing

Stephanie a 40 year old recent mother, experienced perinatal psychosis after the birth of her first child, Tiffany. She has been at the Mother and Baby Unit for four weeks.  As her recovery progresses, you have been instructed by the MDT to begin the process of reintroducing Stephanie to life outside of the MBU, which she has become accustomed to and seems to enjoy.  You have taken Stephanie on leave to the local park and to a nearby coffee shop on two separate occasions.  Stephanie seems hesitant to undertake a home visit though.

Simulation and Learning outcomes

  • To display the cognitive skills to prepare, and communication skills required to engage in a structured conversation planning an escorted visit home with a perinatal mental health patient.

  • To display awareness of the need to assess the environment and the patient’s response to it whilst accompanying a perinatal mental health patient on a home visit.

  • To display the knowledge, skills, and ability to successfully conduct a structured debrief conversation with a perinatal mental health patient on return from escorting them on a visit to their home.

  • To correctly identify and reflect on mitigations for the potential risk factors in a perinatal mental health patient’s home environment, related to both patient, infant and wider family.

Before starting the XR simulation, the learner must consent to taking part via this online form.

The learner will then be given access to the Medical History for the patient, before starting the sim, but will also be able to access it within the sim

🗣️ Discussing escorted home leave

The MDT decided that Stephanie should go on escorted leave home. You have been instructed to discuss this with her. Her baby is in the care of the creche.

The student and Stephanie arrive at her home for her escorted leave. Stephanie is in the bathroom changing Tiffany. This allows you to have a look around her apartment on your own and assess her home situation.

Stephanie has settled her baby to sleep in the bedroom. The student can now begin a debrief with Stephanie.

To wrap up, the student must do a verbal handover of care to the senior clinician on duty who is in their office.

The student, instructor (and possibly other students) will watch a recording of the simulation and discuss in detail the student’s technique and clinical reasoning.

At the end of the training simulation, the learner is invited to complete an evaluation form.
