Stacey Morris After the birth of her second child 4 weeks ago, Stacey’s husband Josh has phoned the surgery as he is very concerned about his wife’s mental state.

The student GP must speak with Josh, decide whether to visit Stacey at home, conduct a Mental Health Assessment and then arrange care for Stacey if they consider it neccessary.

Simulation and Learning outcomes

To be able to take a history from a patient presenting with an acute psychotic illness.
To be able to ascertain and evaluate information relating to safeguarding.
To be able to assess suicide and homicide risk.
To be able to formulate a safe, shared management plan.

Activity	Overview
Briefing	Before starting the XR simulation, the learner must consent to taking part via this online form. The learner will then receive a briefing and be instructed to speak with Stacey. The student GP will be given access to the Medical History for the patient.	Classroom 5 mins
Consultation with Josh	The student GP will conduct a brief phone call with Josh, Stacey’s husband which should lead them to determine that Stacey’s condition requires them to visit her at home	XR Sim GP surgery 2 mins
Mental Health Examination of Stacey	On the home visit, the student GP should conduct a mental health examination of Stacey. The student GP should evaluate risks and note that Stacey is having some signs of psychosis, and had self-harm ideations that are rather well developed.	XR Sim Patient’s home 10 mins
Clinical Decision	The student GP will contact the Mental Health Crisis team who cannot reach Stacey for 48 hours. The most optimal outcome is for the student GP to refer Stacey for a complete mental state examination at the hospital.	XR Sim GP surgery 3 mins
Debrief	The student GP, instructor (and possibly other students) will watch a recording of the simulation and discuss in detail the student GP’s technique and clinical reasoning	Classroom 15 mins
Evaluation	At the end of the training simulation, the learner is invited to complete an evaluation form.
